Two Separate Care Pathways
Phoenix Medical Spa offers two distinct membership programs with completely separate pricing. Choose the pathway that fits your needs.
Primary Care & Urgent Care
Relationship-based, physician-led primary care with access to appropriate same-day and urgent concerns.
There is no monthly household cap. Monthly fees are based on the number of adults ($150/mo each) and adolescents ($98/mo each) enrolled. Enrollment fees are separate one-time charges.
Phoenix Medical Spa PLLC provides primary care for adults and adolescents within the practice's established age range. Families with younger children who require pediatric primary care may be referred to one of our community pediatric partners. Phoenix maintains referral relationships/resources with Cook Children's pediatric practices throughout the North Texas area. Provider availability and insurance participation should be confirmed directly with the pediatric practice.

Medical Weight Loss
Physician-guided medical weight management. Begins with a $397 initial consultation.
- Individualized physician evaluation

Important: Primary-care membership pricing and medical weight-loss pricing are completely separate. Medications, GLP-1s, laboratory testing, and outside services are not included in monthly fees unless expressly stated. Membership is not health insurance.
