Membership is not health insurance and does not replace coverage for emergencies, hospitalization, specialty care, imaging, or services outside the membership agreement.

Select the direct-to-consumer membership that fits your household. Enrollment fees apply separately at the time of enrollment.
Adult Retail Membership
Direct-to-consumer physician-led membership for one adult seeking relationship-based primary and metabolic-health care.
$120 individual enrollment fee

Child / Dependent Membership
Membership for an eligible child or dependent under a participating adult's household.
$120 individual enrollment fee
- Physician-led care for an eligible dependent
- Routine and preventive-care planning
- Virtual follow-up for appropriate concerns
- Communication with the practice
- Transparent, flat monthly fee
Household Membership
Combine eligible adult and dependent rates until the $325 monthly household maximum is reached.
$200 household enrollment fee
- Covers eligible adults and dependents in one household
- Individual member rates apply up to the $325 cap
- One combined household enrollment fee
- Physician-led care for the whole family
- Transparent, capped monthly fee
Household maximum: Eligible household member rates are combined until the $325 monthly household maximum is reached. Our team will confirm your final household structure and amount before charging.
