24-Hour Cancellation Policy: We value your time and the time of our providers. If you need to cancel or reschedule your appointment, we require at least 24 hours' notice. Appointments canceled or rescheduled with less than 24 hours' notice, or missed appointments (no-shows), may be subject to a cancellation fee.

Payment at Booking: Payment is due in full at the time of booking to secure your appointment. This applies to initial consultations and specific treatments. For memberships, your recurring payment method will be charged according to your billing cycle.

Due to the medical nature of our services and treatments, we do not offer refunds on services provided, medical weight loss programs, peptide therapies, or membership fees once charged. Unopened skincare products may be returned within 14 days of purchase for clinic credit. If you have concerns about your treatment, please contact us immediately so we can address them.

HIPAA Notice of Privacy Practices: Phoenix Medical Spa complies with the Health Insurance Portability and Accountability Act (HIPAA). We are committed to protecting the privacy and security of your protected health information (PHI). Your medical records and personal information are strictly confidential and will only be used or disclosed for treatment, payment, and healthcare operations as permitted by law, or with your explicit written authorization.

General Privacy: Any personal information collected through our website or booking portal is used solely for scheduling, communication, and providing medical and aesthetic services. We do not sell or share your personal information with third parties for marketing purposes.

Telemedicine services are provided for specific consultations and follow-ups. While telemedicine offers convenience, it may not be suitable for all medical conditions. By participating in a telemedicine visit, you acknowledge the potential risks, including limitations in physical examination and potential technological disruptions.

If you are experiencing a medical emergency, please call 911 or go to the nearest emergency room immediately. Telemedicine is not intended for emergency medical care.
