Phoenix Medical Spa offers physician-led primary care through a relationship-based membership model, with access to appropriate same-day and urgent concerns when they can safely be managed through the practice.
Membership is not health insurance and does not replace coverage for emergencies, hospitalization, specialty care, imaging, or services outside the membership agreement.
Two Ways Phoenix Primary Care Supports You

Primary Care
Ongoing, relationship-based care including preventive care, chronic-condition management, routine follow-up, health planning, and continuity with your physician.

Urgent / Same-Day Care
Evaluation of appropriate non-emergency acute concerns that can reasonably be managed through Phoenix Medical Spa, such as appropriate minor illnesses or routine acute concerns.

Phoenix Medical Spa is not an emergency department. Some urgent symptoms require emergency or in-person evaluation. If you are experiencing a medical emergency, call 911 or seek immediate emergency care. Membership does not guarantee 24/7 access or guaranteed same-day appointments unless those exact terms exist in your membership agreement.
Choose How You Access Phoenix Primary Care
Two clearly separate enrollment pathways. Pricing is never blended with medical weight loss.
Join Phoenix Directly
Individuals and families who are not enrolling through an employer may join Phoenix Medical Spa directly through our retail membership program.
Adult DPC Membership
$125 one-time enrollment fee
Direct-to-consumer physician-led membership for one adult seeking relationship-based primary care.
Adolescent DPC Membership
$125 one-time enrollment fee
Membership for an eligible adolescent (age 12 or older) under a participating adult's household.
Household DPC Membership
$200 (2–3) · $325 (4+ members)
Adults ($150/mo each) and adolescents ($98/mo each) enrolled together. There is no monthly household cap — your monthly fee is based on the members you enroll.

Have Phoenix Through Your Employer?
If your employer, organization, or benefit program provides access to Phoenix Medical Spa through Hint Connect, enrollment is handled through the Hint platform.

- Phoenix Medical Spa PLLC provides primary care for adults and adolescents within the practice's established age range. Families with younger children who require pediatric primary care may be referred to one of our community pediatric partners. Phoenix maintains referral relationships/resources with Cook Children's pediatric practices throughout the North Texas area. Provider availability and insurance participation should be confirmed directly with the pediatric practice.
